We provide care and support to help adults aged from 16 - 65 years who have been diagnosed as having acute and/or enduring mental health problems who cannot be cared for at home or in the community.
This team is made up of Doctors, Community Psychiatric Nurses, Social Workers, Psychologists and Occupational Therapists.

GPs, A&E doctors, police, liaising team, self-referral, care coordinators and voluntary sector carers can refer to the service.
Who is this service for?
Adults aged from 16 - 65 years who have been diagnosed as having acute and/or enduring mental health problems who cannot be cared for at home or in the community.
What to expect
Our team provide care and support to help patients become well enough to be discharged and where necessary receive care in their homes or in the community. Services include:
- Diagnosis, an appropriate care plan and treatment
- A physical health check on admission
- Support to access advocacy services
We also offer support for physical and social needs.
When an inpatient is ready for discharge this will often be supported by the home treatment team for a period providing additional support whilst working alongside the care coordinator.
