
We provide care and support to help adults aged from 16 - 65 years who have been diagnosed as having acute and/or enduring mental health problems who cannot be cared for at home or in the community.
This team is made up of Doctors, Community Psychiatric Nurses, Social Workers, Psychologists and Occupational Therapists.
Anxiety, Bi-polar Disorder, Depression, Personality Disorder, Psychosis, Psychotic Depression, Schizophrenia
GPs, A&E doctors, police, liaising team, self-referral, care coordinators and voluntary sector carers can refer to the service.

When an inpatient is ready for discharge this will often be supported by the home treatment team for a period providing additional support whilst working alongside the care coordinator.
