DIALOG+ care plan

What is DIALOG+? 

DIALOG+ is a patient led outcomes scale whereby people entering secondary mental health services are asked to rate their satisfaction around quality of life and needs for care on 11 areas:

  1. Your mental health
  2. Your physical health
  3. Your job situation
  4. Your accommodation
  5. Your leisure activities
  6. Your partner/family
  7. Your friendships
  8. Your personal safety
  9. Your medication
  10. The practical help you receive
  11. Your consultations with mental health professionals

DIALOG+ is a facilitated conversation focusing on the lowest scored areas using the DIALOG scale. The Service User and Health Care Professional work together in a strength-based therapeutic way, co-producing a holistic care plan.  

Why use DIALOG+?

  • DIALOG+ enables proactive, personalised conversations which is led by the Service User with their Health Care Professional ,supporting self-management and co-production. This supports Service Users to move forward on their individual journey of recovery.
  • DIALOG+ is being used to help inform the redesign of the care planning processes within mental health services.

As a minimum, it is recommended that DIALOG+ is completed at the start of treatment, during review dates and routine appointments and at the end of treatment.
